MD or DOBoard Certification:
Board Certification in Internal Medicine or Family Practice or experience in Geriatric Medicine.Licensure:
Current active unrestricted license to practice medicine in NY.Experience:Fellowship, and diploma or experience in geriatrics and palliative care is preferred.Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.Schedule:
Monday - Friday 8:00 AM - 5:00 PM (In-Person Clinic, some home care cases once a month)Job Overview:The Physician, under the supervision of the Medical Director, plays a crucial role in providing comprehensive medical coverage and care coordination for participants within an interdisciplinary team (IDT) environment. Essential functions include delivering complex primary care and acute illness treatment, conducting comprehensive assessments, and actively participating in the IDT. The role involves collaboration with case management RNs, contributing clinical input in Transitions of Care settings, and conducting post-hospital and post-ED discharge visits. Additionally, the Physician supervises and supports Nurse Practitioners and Physician Assistants, serves as a liaison between outpatient and inpatient specialists, and conducts visits to assisted living facilities and skilled nursing facilities based on clinical criteria.
