Job Role Summary

This is for an Outpatient Physical Therapist position at our West 38th Street Eskenazi Health Center location. New graduates welcome to apply!

The Physical Therapist provides a wide range of medically prescribed physical therapy assessments and treatments to patients experiencing orthopedic, musculoskeletal and neurological conditions. The Physical Therapist evaluates patient diagnoses, applies diagnostic and prognostic tests to measure muscle, nerve, and joint function, correlates information from the evaluation to develop short-term and long-term goals. The PT will establish a plan of care to restore function and prevent further disability in the presence of disease or following injury/surgery.

Evaluation

Treatment:

Documentation:

Accurately and timely documents patient evaluation and treatment per documentation standardsObtains appropriate signaturesJustifies charges, reflects skilled therapeutic need, functional improvement and progress towards goals

Productivity:

Works at or above established productivity levelVolunteers to take walk-in patientsVolunteers to float to other teams as neededAdjusts schedule according to caseload

Administrative:

Job Requirements

Graduate of APTA approved curriculum.Licensed as a Physical Therapist in State of Indiana or eligible to do so.
