About Your Work

Join our Altais Medical Group Riverside clinic as a

Primary Care Nurse Practitioner , where you'll deliver high-quality patient care within the primary care setting. Your role will involve providing comprehensive care to patients with a range of health concerns, under minimal supervision.

As a Nurse Practitioner, you'll work closely with our clinical team, utilizing evidence-based medicine to manage patient cases. Your responsibilities include diagnosing and treating patients, prescribing medications, and coordinating care services as needed. We seek individuals with a strong commitment to patient-centered care and a passion for promoting health and wellness.

In this role, you'll advocate for patients' needs, ensuring they receive timely and cost-effective care. You'll also play a key role in promoting effective communication and collaboration among healthcare providers and patients. Additionally, you'll have the opportunity to contribute to clinical initiatives aimed at enhancing patient outcomes across all Altais Medical Group clinics.

You Will Focus On: Obtain history and perform physical examination. Complete review of medicines and related instructions. Review home safety and psychosocial needs. Fulfill documentation needs for quality and HCC measures. Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions. Instruct all patients, caregivers, and family to call you prior to accessing medical care, subject to life threatening emergencies. Maintain patient safety and confidentiality. Universal precautions utilized in all patient care. Prescribe/order procedures, treatments, and medications within scope of practice. Develop high level of trust and intimacy with patients and families. The Skills, Experience & Education You Bring

Master of Science in Nursing (MSN) degree or Doctor of Nursing Practice (DNP) degree. Active California Nurse Practitioner License, National Provider Identifier (NPI) and Drug and Enforcement Administration (DEA), all in good standing. Minimum of one year of experience in primary care clinic. Demonstrated knowledge and experience using electronic medical records, Epic experience strongly preferred. Experience providing evidence based and value-based medicine, serving as patient health advocate. Working in interdisciplinary teams. Applying quality improvement plans. Ability to function independently as well as part of a team in a dynamic environment. Excellent communication skills and ability to adjust communication styles when working with patients, their families, and colleagues. Highly organized, strong time management skills and flexibility to manage multiple timelines and activities. You Share our Mission & Values

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of workday.
