Full-time

Homeless Outreach Case Manager , Monday through Friday from 9am to 5pm. Utilizing the Housing First model, provide outreach, engagement, and case management services to homeless individuals in Portage Park and surrounding neighborhoods. The position will help connect clients to resources to increase access to housing as well as increased community functioning and independence, and to decrease crisis episodes, hospital admissions, and police encounters. Homeless individuals may also be experiencing mental health or substance use challenges, and the case manager will connect individuals to services to stabilize them as well as provide counseling.Benefits and Perks:On Demand Flexible Paydays for earned wages through an app called Dayforce Wallet.Competitive salary based upon relevant education, experience and licensure.Salary $22-$23/hour.Opportunity for advancement.Comprehensive benefits package for Full-Time employees includes healthcare insurance, up to 26 days of paid time off per calendar year, 11 paid holidays, sick time, 403(b) plan, Employee Assistance Program, and flexible hours.The paid training you need to learn, grow, and succeed!Essential Functions:Provide outreach and engagement services to unhoused individuals.Provide and coordinate a continuum of housing location and case management/treatment services.Ensure all documentation is completed in a timely manner and meets Rule 140/132 and/or MCO requirements.Transportation of clients and community visits are required. Services will be provided in multiple locations.Comply with documentation and productivity standards.Adherence to ethical standards of treatment according to NASW Code of Ethics.Carry a caseload of clients providing supportive counseling and concrete services assigned by Supervisor and collaborate with other internal and external programs and treatment providers in order to provide these services.Position Qualifications:High School Diploma required. Associates, Bachelors or Masters preferred.LSW/LPC, LCSW/LCPC preferred.1-3 years of experience working with mental health and/or substance use population is preferred.1-3 years of experience providing outreach and engagement services to unhoused individuals is preferred.Proficiency in EHR documentation and review.Familiarity with accreditation and licensure standards.Ability to work in a team environment.Excellent written and verbal communication skills.Excellent communication, organization, presentation and PC/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.Valid Illinois statutory and liability insurance coverage is required.Valid Driver's License in good standing for the state of residency and insurance.
