OCCUPATIONAL THERAPISTQUALIFICATIONS: A current and valid Illinois license issued through the Illinois Department of Professional Regulation as a Registered Occupational TherapistREPORTS TO: Director of Special EducationJOB GOAL: The Occupational Therapist provides educationally relevant diagnostic and therapy services as a multidisciplinary team member.
PERFORMANCE RESPONSIBILITIES:
Collaborates with educational personnel to enable a student to meet his/her IEP goals and objectives.Monitors student performance within the classroom.Completes evaluation or other screenings as requested by the educational team.Completes diagnostic evaluations to determine the functional skill level of students and presents the results of the evaluations at multidisciplinary conference.Generates initial, re-evaluation and annual progress reports.Participates as a member of the multidisciplinary teams that determine eligibility for special education services.In conjunction with the IEP team, develops annual goals and objectives for students who require services and updates students goals and objectives as required.Participates in team meetings and staffings as directed.Implements educationally relevant services for assigned caseload.Ensures that physician's orders, information releases, school records, evaluations, and other correspondence are on record and reviewed.Creates and follows a weekly work schedule and gives a copy of the schedule to the supervisor.Maintains a written inventory of equipment and supplies used in programming and recommends supplies and equipment as needed.Maintains a calendar record of all therapy services provided to each student and prepares Medicaid reports as needed.Maintains equipment and materials in proper working condition.Monitors the progress of students and makes adjustments to treatment plans as needed.Provides consultation and in-service training to staff, parents, and other professionals as directed.Establishes and maintains effective working relationships with local district and cooperative staff.
EVALUATION: Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Professional Personnel.
