Position Summary:
The Physical Therapist is a licensed professional responsible for evaluating, planning, and implementing physical therapy to the residents and the community as prescribed by the attending physician. The Physical Therapist works as a member of an interdisciplinary rehabilitation team, which oversees each resident's physical therapy and restorative plan of care.
Qualifications:
Graduate of a state-approved School of Physical Therapy accredited by the American Physical Therapy AssociationTwo years of long term care or rehabilitation center experience preferred. Outpatient therapy experience may be considered if practice included medical/surgical patients.Licensed as a Physical Therapist in the Commonwealth of PennsylvaniaCPR certification strongly preferred and encouragedEssential Duties and Responsibilities:
Performs a comprehensive evaluation of residents/patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability testsDevelops the plan of care cooperatively with the physician, resident/patient, family and other team members with revisions made based on patient conditionProvides therapeutic actions to relieve pain and/or develop or restore/regain maximum independenceInstructs residents/patients, families, nurses, and nursing assistants in the total physical therapy program including transfer activities, ambulation skills, exercise program, the proper and safe use of prosthetic and/or HEP maintenance programPlans with the resident/patient, family, rehabilitation members, and nursing for dischargeEstablishes and implements restorative/maintenance Range of Motion and walking programs in conjunction with nursing, and assists nursing with evolving changes in restorative programsSupervises Physical Therapy Assistant(s) in the implementation of the treatment planConducts screening of residents at regular intervals to determine the need for intervention/treatmentDocuments results of assessment, treatment, follow-up, discharge, and billing notes in an accurate, timely manner in accordance with Standards of Practice, state and federal regulations, and policies/proceduresCommits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.Benefits:
