Statewide Support Services Coordinator

The New York Association on Independent Living (NYAIL) is seeking a Statewide Support Services Coordinator to assist in the development and delivery of Individualized Service Plans and Support Services through the Olmstead Housing Program (OHS) and Rapid Transition Housing Program (RTHP). The housing programs are designed to provide rental subsidies and supportive housing services to high-cost, high-need Medicaid members in New York State. The housing programs are funded by the NYS Department of Health. NYAIL administers the grant statewide and subcontracts with its member Independent Living Centers (ILCs) across the state to staff Independent Living Specialists.

NYAIL is a state-wide, non-profit organization, working with Independent Living Centers across the State to eliminate physical, communications, attitudinal, and other barriers for people with disabilities. NYAIL members are Independent Living Centers (ILCs), consumer controlled, cross-disability providers of advocacy, services and supports to people with disabilities.

Responsibilities:

The Support Services Coordinator (SSC) will work in collaboration with the second Support Services Coordinator (SSC) and will report to the Director of Housing Programs to facilitate the successful transition of individuals into community settings across all areas of the State.

The SSC will be responsible for coordinating activities of the Independent Living Specialists (ILS) at subcontracting Independent Living Centers (ILCs) in the identified service areas.

Responsibilities include, but are not limited to:

Work collaboratively with NYAIL’s housing team on the development, evaluation, and ongoing supervision of this subsidy program.

Overall responsibility for the development, delivery and evaluation of Individualized Service Plans (ISP) to meet the needs of housing program participants.

Provide ongoing support to Independent Living Specialists at subcontracting ILCs and oversight of the provision of support services to current and new housing program participants.

Development and implementation of a plan to identify, verify, enroll, and document eligible participants in need of housing assistance and ILS services.

Develop and evaluate reports to meet work plan objectives including but not limited to tracking outcomes of ISPs through progress notes and ISP renewals. Identify trends and provide feedback to ILSs and Directors.

Provide technical assistance and support to ILSs throughout the state through technical assistance calls.

Work at the State level and locally with subcontracting ILCs to establish mutual referral systems with Managed Care Organizations, Performing Provider Systems, hospitals, nursing homes, service providers, etc.

Plan and coordinate regional, and web-based training events, as needed.

Ensure compliance with all New York State Department of Health reporting requirements, including data collection.

Develop program materials (e.g., program procedures, enrollment documents, and marketing materials) as needed for successful implementation.

Complete regular audits on ISPs to ensure person-centered principles are applied to each participant with a concentration on outcomes being achieved.

Qualifications:

Minimum of a bachelor’s degree, Master’s degree preferred

Experience with the concepts of Independent Living, consumer control, and a strong commitment to promoting and advocating for the full integration of individuals with disabilities.

Experience with developing individualized service plans using person-centered principles.

Experience transitioning individuals including familiarity with finding affordable, accessible, integrated housing and the development of individualized service plans.

Knowledge of state and local housing policies, practices, laws, and issues impacting seniors and people with disabilities strongly preferred.

Knowledge of community-based supports with a strong ability to locate and connect with service providers across the state.

Experience designing, implementing, and evaluating health-related programs.

Knowledge of the state’s Medicaid program and community resources which can assist people with disabilities in living independently.

Excellent interpersonal, organizational, computer, and communication skills.

Personal experience with disability is a plus.

Salary and Benefits:

NYAIL offers a competitive salary and generous benefits package. The salary estimate for this position is in the $58,000- $62,000 range.

Location and Travel:

This is an Albany-based position with remote work available. Regional and statewide travel required.

This is a grant funded position through October 31, 2029, with possibility for extension contingent on additional state funding.
