Description
The Support Coordinator manages the Community Habilitation program. They serve as a liaison to the families of children with disabilities and the care provider. It is a rewarding and elated feeling to help individuals and their family navigate daily challenges and working towards a better future. Successful coordinators possess the following qualities; outgoing, energetic, positive, friendly person and have the ability to build relationships with both clients and staff. The job requires the coordinator to be caring, job-oriented, and focused.
Responsibilities:
Interview, hire, train, supervise, evaluate, and manage program staff
Liaison between families and care providers
Maintain charts and manage program time sheets submissions
Write and implement staff action plans
Provide coverage with participants in the program in the absence of program staff
Ensure compliance with OPWDD and Medicaid regulations
Complete incident reporting process as per requirements
Maintain thorough documentation of services
Requirements:
Excellent proficiency with computers
Strong organizational skills.
Strong interpersonal skills.
Excellent oral and written communication skills
Able to communicate effectively with staff, management, and other outside personnel.
Arabic or Chinese speaking a plus
Education
Bachelor's Degree in health-related or human services with one year of experience working with people with developmental disabilities.
