We’re hiring a Wound Care LPN at The Grand Rehabilitation and Nursing Center at Barnwell (located in Valatie, NY)
Position Summary:The Wound Care LPN plays a crucial role in managing, treating, and the prevention of skin conditions for our residents. in collaboration with physicians, and the Wound Care RN, you will provide hands-on care focused on wound treatment and maintaining skin integrity while ensuring that residents receive the highest standard of care.
Reports to the Director of Nursing.
Key Responsibilities:Wound Care Assistance & Treatment
Provide care for residents with skin conditions and wounds, ensuring proper treatment and wound management.
Implement wound care treatments and interventions.
Maintain and update resident care plans.
Skin Integrity & Wound Management
Monitor the condition of residents' skin and assist in providing preventative care to maintain or restore skin integrity.
Identify and report any changes in residents’ skin conditions or wounds.
Ensure proper documentation of wound care activities, treatments, and progress.
Collaboration & Documentation
Work closely with the Wound Care RN and other healthcare providers to ensure coordinated care.
Assist in maintaining accurate documentation of treatments, assessments, and wound care interventions in accordance with facility protocols and regulations.
Communicate effectively with residents, families, and the healthcare team to support quality care and outcomes.
Qualifications:
Current New York State LPN license.
Nursing degree from an accredited college or university, or completion of an approved LPN program.
Previous experience in long-term care or wound care settings is a plus.
Knowledge of basic wound care principles and skin integrity management is preferred.
