Harborview Medical Center's (HMC) Pediatrics Youth's clinic has an outstanding opportunity for a part-time FNP, Family Nurse Practitioner; or PA-C, Physician Assistant.
This position represents an exciting opportunity to expand healthcare services to unstably housed youth. Harborview is committed to ending youth homelessness and closing paths to chronic adult homelessness. Working with community partners, we will address physical and mental health challenges to allow unhoused youth to engage with education, employment, and stable shelter.
This is a position for an experienced advanced practice provider ready to work with some of the most marginalized and resilient young people in our community. You will be an integral part of clinical care as well as the ongoing growth and design of our service delivery model. The role calls for creativity, flexibility, dedication and compassion.

WORK SCHEDULE:
* Part-time, 28 hours per week* Two clinical shifts are early morning (0700-1100) at a shelter site in the University District.* Two clinical shifts will be late afternoon/evening (1600-2000) at a shelter site in Auburn.* Time is credited for travel to offsite locations.
DEPARTMENT DESCRIPTION:
Harborview's Youth Clinic provides primary care, acute care, reproductive healthcare, and mental health services to teens and young adults (12-26 years old) who are unstably housed or experiencing homelessness and seeking services at one of 2 partner social service agencies. Clinic facilities are basic, and providers are routinely called upon to use clinical acumen and creativity to assess and meet client needs. The staff include a seasoned program manager and a mental health clinician. Child & adolescent psychiatric consultation is available as needed. And the clinic works closely with other units at Harborview to access laboratory testing, pharmacy, billing and documentation services.

PRIMARY JOB RESPONSIBILITIES:
* Function independently as an expert practitioner, consultant, educator, and researcher within Pediatrics.* Function within the scope of licensure and in accordance with department, hospital, and university policies/standards.* Elicit comprehensive health histories from teens and young adults; screen for health risk behaviors and common mental health conditions, with the ability to interpret and act upon screening results; order and interpret common laboratory and other diagnostic tests; diagnose and treat common acute illnesses and minor injuries.* Provide reproductive health care, including sexually transmitted infection screening and treatment, contraception management, and preexposure prophylaxis for HIV.* Order immunizations and perform physical examinations; provide anticipatory guidance and health advice; follow evidence-based clinical practice guidelines for screening and treatment; recognize and respect limits of confidentiality for teen patients and emancipated minors; medication management and follow-up (with psychiatric consultation if needed) for common mental health concerns; interview for behavior change related to health risks and substance misuse.* Performance of and interpretation of point of care testing and phlebotomy; able to place Nexplanon (or similar LARC) or willingness to learn and perform this skill; may perform minor surgical procedures, such as suturing lacerations and incision and drainage of abscesses, including administration of local anesthesia.* Document pertinent findings & diagnostics on chart and/or in dictation, complete Pro Fee sheet appropriately; provide all data required for performance metrics required by grantors; order appropriate diagnostic tests; follow through with diagnostic tests; make discharge referrals appropriately; prescribe appropriate medication on an individualized basis; request appropriate specialty consults; order PT, OT, Diabetic Education, etc. appropriately.* Function as the clinical lead for this community-based service; work collaboratively with the program manager to develop clinical protocols, workflows, and care standards specific to the population served; represent the clinical program and Harborview Medical Center to shelter staff and other partners in the community; participate in grant planning, data collection, and grant reporting activities as required to maintain funding.* Provide direction for creative approaches to the management of complex patient problems; provide educational and support to physician assistants, nurses, peers, students residents and the community.* Participate in interdepartmental efforts to facilitate a safe environment; understand and follows hospital and departmental protocol for emergencies and disasters; take immediate action to address environmental, equipment or engineering issues that present a potential safety risk.
REQUIREMENTS:
* Family Nurse Practitioner:
Master's Degree in nursing from an accredited school of nursing
AND Washington State Advanced Registered Nurse Practitioner License with prescriptive privileges
AND National certification as a Family Nurse Practitioner
AND ACLS certification.
* At least 2 years of experience as an FNP, preferably with the unhoused population.
OR
* Physician Assistant:
Licensure as a Physician's Assistant in the State of Washington with national certification from a national certifying body (NCCPA).
Applicant must be eligible to receive hospital staff appointments for adjunct staff membership by the Organized Medial Staff of the hospital under Medical Staff By-Laws for hospital-based positions.
AND ACLS certification
* At least 2 years of experience as a PA, preferably with the unhoused population.
