Eagle Physicians Internal Medicine at Tannenbaum is seeking an experienced and detail oriented Nurse Health Advisor to join our team. Our Nurse Health Advisors are Certified or Registered Medical Assistants or Licensed Practical Nurses. Occasionally an RN can fulfill this role but is generally overqualified. This individual is a key member of the primary care team, serves as a clinical health advocate and works with a targeted group of patients to support the primary care team in such a way as to ensure physicians are operating at the top of their licenses while ensuring patients are receiving the preventive care they need. The Nurse Health Advisor will help individual patients develop behavioral changes to improve health outcomes such as medication adherence and chronic disease management. They work toward improving overall quality metrics ratings by bringing members into compliance and closing gaps in care. The ideal candidate must be friendly, approachable and self-confident. They must have excellent verbal communication skills, an open-minded perspective to working with diverse populations, problem-solving skills, and a strong desire to help. M

inimum of 3 years prior experience in a medical office is strongly preferred.

Eagle Physicians may pay all support staff employees hired as a .50 FTE or higher on-or-after April 1, 2024, structured bonuses over their first two years of employment with Eagle as outlined provided. Bonus Amounts are $750.00 at 6 months, $750.00 at 12 months, $1,000.00 at 18 months and $1,000.00 at 24 months.

Paid Maternity/Paternity leave- Eagle provides up to 2 weeks of paid maternity/paternity leave after 1 year employed and 1,250 hours worked for the birth or adoption of a child. Time is pro-rated based on FTE status.

Schedule: Generally, Monday - Friday 8 AM-5 PM. Schedule subject to change as indicated by manager

Key Responsibilities:• Attends required meetings and participates in committees as requested. Participates in professional development activities and maintains professional affiliations. Functions as a member of the interdisciplinary team, providing updates on trends and outcomes to the provider.• Pre-Visit Planning: Identify patients who have an upcoming appointment, identifies gaps in care. Initiate standing orders as appropriate.• Initiate standing orders, lab orders and refills as appropriate.• Annual Medicare Wellness Visits (AMW): Identify and possibly schedule patients who are eligible for a Medicare Annual Wellness Visit and educate these patients on the importance of these visits. Conduct AWVs. Refer patients to physician as appropriate. Completes and documents information in Electronic Medical Record (EMR).• Understands system quality improvement expectations (MU, ACO, and PCMH) and works to align the provider's panel with those metrics.• Reviews and interprets provider level data on QI activities including evaluation of the effectiveness of interventions, adjusting plans as needed and presenting outcomes to provider and practice manager.• Review and identify patients in need of screenings, tests, vaccines, and immunizations.• Work with schedulers to schedule appointments for patients.• Conduct visits to provide necessary screenings, tests, vaccines, and immunizations.• Document appropriately in the EMR.• Activate Chronic Care Management coordination if indicated.• Collect and review pharmacy reports to determine whether patient is adhering to medication regimen.• Work with patient to identify and resolve any barriers to their medication adherence.• Review and verify list of medications and compare to EMR data, make changes and necessary• Operate medical equipment to include but not limited to: blood pressure cuff, thermometer, needles, syringe and EKG.• Maintain neat and clear work areas and restock supplies as needed.• Make sure restricted or abusable materials to include drugs, needles, syringes, prescription pads and patient medical records are secured and accessible only to authorized medical office personnel.• Follow clinic policies relative to sample prescriptions; dispensing only upon signed authorization of a provider.• Dispose of infectious waste in accordance with training procedures.• Attend required staff meetings and participate in committees as requested.• Maintain exam rooms for necessary supplies and materials. Ensure cleanliness.
