The
Occupational Therapist
(OT) provides occupational therapy evaluations and treatment plans to participants. All participant care provided is in accordance with applicable InnovAge policies and state regulations.Provides clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.Assesses condition, creates a treatment plan and discusses implementation of therapy with participant or caretaker(s).Integrates occupational therapy treatment plan in the team Plan of Care (POC).Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment for participants.Makes home visits/educational sessions as needed and indicated in the POC, and documents activities including participant/caregiver response in participant's chart.Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of durable medical equipment (DME).Administers treatment program per referral from assessments from the interdisciplinary team (IDT)Instructs family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes.Evaluates charts and reports on participant's progress to appropriate staff.Documents occupational therapy services provided and performs department record-keeping in keeping within documentation requirements.Performs data collection, prepares reports and maintains any necessary participant statistics.Assists in recommending policies and procedures in the area of occupational therapy.Assists the physician or intermediate care provider in evaluating level of function, helps develop the plan of care and revise as necessary.Participates in in-service programs and presents in-service programs as assigned.Participates with all other health care personnel in patient care planning.Performs all skilled occupational therapy procedures as ordered by physician.Writes reports to physicians regarding patient's progress.Attends all staff meetings and patient care conferences as scheduled.Completes a discharge summary upon completion of therapy services.Prepares and submits patient evaluations, clinical and progress notes in a timely manner as outlined in Agency policy.Immediately reports any patient incidents/variances or complaints.Participates in peer review and QM activities as requested.Participates in utilization review of medical records as assigned.Adheres to Agency standards and consistently interprets and accurately performs all assigned responsibilities.Reports all incomplete work assignments to appropriate member of clinical management staffWork in collaboration with InnovAge nursing home and assisted living staff regarding the occupational therapy needs of participants who are their residents.Conduct and participates in clinic educational programs.Performs data collection, prepares reports and maintains any necessary participant statistics.Maintains therapy department in a clean and orderly manner and oversees the maintenance of therapy equipment.Maintains the quality assurance, safety, environmental and infection control programsFulfills the documentation requirements and department record-keeping of OT services provided as set by InnovAge programsAttends Morning Meetings, mini teams and family meetings and forwards all information to the Primary OT if no other OT is available or if assigned by the Primary OTParticipates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants.Performs other duties as assigned.REQUIRED
Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA)Bachelors in (OT), Masters in OT (MOT), Doctorate in OT (OTD)National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.Registration with state in which practicingRequires state issued driver's license, personal transportation, good driving record and auto insurance as required by lawA minimum of one year experience working with the frail or elderly is required.PREFERRED
Two (2) years' experience, preferredA minimum of one year of general occupational therapy experience in a geriatric setting preferred
