Coverage: ASAP – 3-6 Months, with option to extend Shift Schedule: Saturday and Sunday 8:30 AM – 5:00 PM plus Wednesday, & Thursday 8:30a-7:00p. 36 Hours / week No call Patient Types: Well Child Physicals (9-12 years), well baby & child visits, immunizations Patients per shift: Estimated 25-28 Patients per 8 Hour Day Support Staff: Full support staff – RN’s, MA’s, LVN’s EMR: Epic Credentialing timeframe: 12 Weeks from the time application submitted Requirements: Active unrestricted CA License Active DEA Active AHA BLS certifications Minimum 3 years of experience Board Certified Full-time coverage is mandatory—no deviations from the requested schedule will be accepted. Must be able to provide at least 2 supervisory references.
