Virtual EMR training must be scheduled prior to the providers first day, to avoid the provider missing their first day. Travel Preferences: Air Travel: Acceptable; Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations Credentialing Information: Timeframe: 30 days Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? Yes Emergency privs? Yes Requirements: Active Unrestricted CO license - Required or IMLC with LOQ in hand at time of submission Board Certified OBGYN - required Active BLS - required Active DEA - required (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.
