Job Title: Case Manager Location: Greater Los Angeles, CA Company: LHH Recruitment Solutions (on behalf of our nonprofit client) Job Type: Full-time, Contract, 100% Onsite (with some field work) About the Role: LHH Recruitment Solutions is seeking a compassionate and dedicated Case Manager to join our nonprofit client’s team in the Greater Los Angeles area. This role is integral to supporting program participants in securing stable housing by providing immediate medical attention (both mental and physical), assisting with necessary documentation, and guiding them in setting long-term goals. This position requires a hands-on approach, working closely with individuals who may be homeless and dealing with mental health or substance abuse challenges. Key Responsibilities: Assist clients in accessing medical care, both mental and physical, as needed. Help participants obtain necessary documentation (e.g., ID, social security, etc.). Support clients in setting and achieving long-term housing and personal goals. Provide crisis intervention and de-escalation for individuals experiencing high-stress situations. Conduct fieldwork as required to support participants in various environments. Maintain accurate and timely case records within HMIS and other software systems. Work collaboratively with a multidisciplinary team to provide holistic support to clients. Qualifications: Bachelor’s Degree required. Experience working with individuals who are experiencing homelessness Prior case management and/or social services highly preferred. Must possess a valid Driver’s License. Proficient in Microsoft Office. Skills: Strong empathy and compassion while maintaining professional boundaries. Excellent written and verbal communication skills. Ability to de-escalate high-pressure situations calmly and effectively. Capable of working both independently and as part of a team. Adaptability in using various software systems. Why Join Us?
