Scheduler/Office Assistant

We are seeking a highly organized and detail-oriented individual to join our team as a Home Health and Hospice Scheduler. The ideal candidate will be responsible for coordinating and scheduling the delivery of home health and hospice services, ensuring that patient care is efficiently and effectively managed. This role requires excellent communication skills, attention to detail, and the ability to work collaboratively with healthcare professionals, caregivers, and administrative staff.

Responsibilities:

• Scheduling and Coordination:
• Schedule home health and hospice services for patients based on their care plans.
• Coordinate and organize daily to ensure optimal patient care.
• Communication:
• Maintain regular communication with caregivers, and patients to confirm appointments and address any scheduling concerns.
• Ensure compliance with regulatory requirements and organizational policies related to scheduling and documentation.
• Problem Solving:
• Anticipate and resolve scheduling conflicts or issues in a timely and efficient manner.
• Collaborate with various departments to address any challenges that may arise during the scheduling process.
• Monitor and evaluate the effectiveness of scheduling processes, identifying areas for improvement and implementing solutions.
• Administrative duties as assigned

Qualifications:

• High school diploma or equivalent
• Previous experience in scheduling
• Proficiency in scheduling software and Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, Wellsky)
• Strong attention to detail and organizational skills
• Ability to multitask and prioritize tasks effectively
• Must be proficient in computers
