Hospitalist Physician

Responsibilities: * Provide oversight and mentorship to our clinicians in the greater Greenville, North Carolina, area * Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care * Represent TeamHealth in engaging, establishing, and maintaining relationships within the community * Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees * Collaborate with leadership to review care coordination initiatives * Develop intervention plans, and share ideas about network management issue * Work alongside pharmacists on medication management * Complete Medicare required visits * Complete acute, follow up, and discharge visits as necessary * Implement an individualized plan of care for patients' goals * Make recommendations on transitional care, DME, home health services, or outpatient services after short stay * Facilitate and assist leadership in quality improvement initiatives and risk management * Timely response in chart completions and pharmacy recommendations Clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), geriatric, primary, internal (IM), adult, post-acute (PAC), home health, hospice, palliative and hospital medicine (HM).
