Opportunity Overview: * Uncapped productivity income potential with base * Average 6-8 hours daily rounding * Leadership stipend Responsibilities: * Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facility * Provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrollees * Work alongside pharmacists on medication management * Complete acute, follow up, advanced care planning, discharge, and wellness visits as necessary * Implement an individualized plan of care for patients' goals * Make recommendations on transitional care, DME, home health services, or outpatient services after short stay * Timely response in chart completions and pharmacy recommendations Experience as a physician in a skilled nursing facility preferred. However, physicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), internal (IM), emergency (EM), primary, adult, post-acute, home health, hospice, and palliative medicine. Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $193,000 to $333,00 annually with no cap on productivity income potential.
