a competitive total compensation package!* Adult and Aging Social Work Supervisors in the Home and Community-Based Alternatives Waiver (HCBA) Program review and approve ongoing case actions, consult with social workers to provide feedback and direction, and review cases submitted for closure, ensuring appropriate case actions have occurred within program and state policies, procedures, mandates, and timelines. They will also supervise and guide a team of Adult and Aging Social Workers, ensuring that each case is managed effectively, ethically, and in full compliance with state and federal regulations. Additional responsibilities include:
Equitably and effectively planning, organizing, and supervising social worker performance Making critical, independent decisions regarding case management, service planning, and resource allocation, ensuring client needs are met in alignment with HCBA Waiver program standards Engaging with community partners and representing HCBA in meetings, such as Multi-Disciplinary Team meetings and Department of Health Care Services (DHCS) Waiver meetings Participating in on-going program development and policy and procedure protocol to ensure compliance with best practices Performing a variety of supervisory tasks, including evaluating and training staff, explaining staff needs to administration, and facilitating meetings with staff and community partners Holding group and individual meetings to review work activities for appropriateness of services and referrals Meeting, consulting, and collaborating with other HSD supervisors and partner agencies to coordinate work Providing back-up support to other Adult and Aging supervisors and their units as needed The ideal candidate for this position will possess many of the following:
A Master's degree in Social Work or a social services related field Experience supervising, training, and/or coaching staff The ability to manage difficult or challenging conservations with personnel, clients, or others in the community Experience working with seniors, adults, and/or children with complex medical needs in a home setting Knowledge of social work principles and mental health conditions for adults who are older and people with disabilities Experience working with regional centers, hospitals, rehabilitation centers, skilled nursing facilities, or other institutions or medical settings Knowledge of community resources, crisis intervention techniques, risk assessment, and task prioritization Experience working with community agencies and advocating for adults who are older and people with disabilities The ability to manage competing priorities in a fast-paced work environment An understanding of Trauma-Informed Care and Diversity, Equity, and Inclusion (DEI) Excellent interpersonal and communication skills, and the ability to interact with a diverse client population The ability to interpret and apply policies, procedures, and laws Knowledge of HCBA Waiver programs, Medicaid, and relevant state and federal regulations is preferred About the Adult and Aging Division (A&A) The Adult and Aging Division assists and advocates for the safety, health, dignity, and independence of adults who are older and persons with disabilities. A&A focuses on protective, supportive social services, as well as community advocacy and coordination. The Division encompasses several programs serving adults who are older and people with disabilities including the Veterans Services Office, Public Guardian, and In-Home Supportive Services (IHSS). The Division also includes the Sonoma County Area Agency on Aging program, and its Advisory Council, which plans, coordinates, and allocates Older American Act funds for services for persons 60 years of age and older in Sonoma County. Minimum Qualifications Education and Experience:
Any combination of training and experience which would provide the opportunity to acquire the knowledge and abilities listed. Normally, a Master's Degree in social work, or a Master's Degree from a two-year counseling program, such as marriage and family therapy, clinical counseling, mental health counseling, addiction counseling, and counseling psychology; and at least two years of experience comparable to Adult and Aging Social Worker;
OR,
Graduation from college, with a Bachelor's Degree in social work, sociology, psychology or other behavioral science and at least four years of experience comparable to Adult and Aging Social Worker or two years as a Social Worker Supervisor I overseeing cases involving older or dependent adults and/or people with disabilities or complex medical conditions;
OR,
Academic coursework at a college or university in social work, sociology, psychology or other behavioral science and at least seven years of experience comparable to Adult and Aging Social Worker would provide such opportunity.
License:
Possession of a valid driver's license at the appropriate level including special endorsements, as required by the State of California, may be required depending upon assignment to perform the essential job functions of the position. Knowledge, Skills, and Abilities Thorough knowledge of:
