Position Summary:
Collaborates with Medical Center Physicians, residents and other professional staff to deliver quality palliative care services to hospital patients under the direction of the palliative care physicians.

Available as a consultant to patients, families, and staff regarding palliative care questions
a. Serves as source of information on palliative care services and symptom management techniques.
b. Promotes palliative care practices among physicians, residents and other staff.
c. Works in the community to increase awareness and advance the practice of palliative care medicine.

Efficiency and Effectiveness
Delivers palliative care education
a. Provides education on palliative care services to clinic staff.
b. Collaborates with outpatient providers and staff to identify palliative care education needs.
c. Participates in the development of continuing education conferences for staff on end-of-life care and other programs related to palliative care medicine.
Participates in interdisciplinary team meetings to review current patients
a. Attends interdisciplinary team meetings.
b. Collaborates effectively with palliative care team members (including physicians, chaplain, SW, music therapist), to establish palliative care interventions.
Quality and Safety
Coordinates and completes palliative care consults
a. Manages assigned patients daily.
b. Documents assessment and plan in a timely manner and discusses patient issues with referring physician as needed.
c. Collaborates with clinic staff for effective delivery of patient care.
d. Completes appropriate billing on assigned patients in a timely manner.
e. Collaborates with pharmacy on utilization of medications and equipment in the delivery of care.
Delivers palliative care services
a. Follows palliative care policies and procedures.
b. Collaborates effectively with supervising physicians.
c. Completes medical staff credentialing requirements.
Evaluation
a. Monitors the achievement of clinical outcomes related to patient plans of care and communicates issues/variances with physicians and other care team members.
b. Assists in quality improvement initiatives.
Participates in implementing appropriate measures of patient care quality and satisfaction
a. Engages in plans to measure quality of palliative care services.
b. Participates in activities to ensure and measure patient and physician satisfaction.
c. Assists with tracking care of patients referred to palliative care.
Performs Analysis
a. Evaluates and analyzes assessment data. Identifies patient(s) needs on initial assessment and throughout the patient's care.
b. Makes necessary referrals as appropriate.
c. Clarifies goals of care and monitors progress to goals. Intervenes as appropriate.
d. Evaluates developmental and mental status and utilizes data to modify plan of care as needed. Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate safe care.
e. Screens data, lab tests, diagnostic or treatment progress of patients in order to make appropriate care team referrals.
f. Demonstrates ability to make complex decisions and the ability to synthesize patient clinical data, diagnosis co-morbidities and complications and facilitates medical records documentation reflective of care plan and treatment.
Position Qualification:
Masters Preferred
Or equivalent combination of education and experience.
