LPN- NEW OPIOID CLINIC
Horizon Health is seeking a LPN for our clinic in Highpoint, NC. The OTP LPN delegates, coordinates, and evaluates patient care through collaborative efforts with the interdisciplinary team members and in accordance with the nursing process and the OTP standards of care and practices.
RESPONSIBILITIES:
- Dispenses Methadone and other medication, per physician orders and per clinic policy
- Accurately record all patient information in the patient record
- Completes brief assessment of patient's mental and physical well-being, monitoring for signs/ symptoms associates with licit and illicit drug use
- Participates in UDS surveillance, per policy
- Coordinates medication reviews and staffing with agency physicians
- Completes all required controlled substance inventories to ensure accurate medication counts
- Conducts patient education programs according to needs identified, including health and wellness education
- Participates in treatment team meetings, providing feedback to the interdisciplinary team

- Current NC LPN license, Bachelor's degree preferred
- Minimum 2 years experience in an addiction care setting (Inpatient, Detox, Partial Hospitalization, OTP) preferred.
