Housing Case Manager
Position Salary: $22.00-$25.00 Job Type: Regular Full Time Department: Community Services Work Location: 1111 Cornwall Open Date: 11/27/2024 Use your case management skills and experience working with mental health and substance use disorders to support our community's most vulnerable population in improving their lives. The Housing Case Manager provides direct case management services to individuals and families who are homeless or at risk of becoming homeless, focusing on helping them achieve housing stability. We use Housing First, Harm-Reduction, and Trauma-Informed models of case management that are client-driven, solution-focused, and facilitate change and movement toward self-sufficiency. We frequently provide support to households living with mental health conditions and/or substance use disorders. Specific duties may include, but are not limited to, home visits, intakes, tracking of services, and assisting in program development. This position is full-time in the office or in the field working with clients. This position offers three different full-time schedule options after the introductory training period including: Standard: Monday-Friday 8am-5pm (8 hrs) Flex: Monday-Thursday 7:30-5pm (9 hrs), Friday 8am-12pm (4 hours) Compressed: Four 10-hour shifts 7am-5:30pm, with Mondays or Fridays off This position starts between $22.00-$25.00 per hour dependent on experience. Annual increases provided (dependent on funding and performance). Must have a valid driver’s license. ESSENTIAL FUNCTIONS/RESPONSIBILITIES: Provides homeless housing-related case management services to families participating in homeless housing programs. Conducts intakes and provides assessments. Maintains confidential case notes on each individual or family client in a timely manner. Develops and maintains cooperative working relationships with other agencies and service providers. Serves as a link between client and resources. Provides reporting and tracking information for data management systems. Determines client eligibility for homeless housing services. Ensures that all federal, state, agency and/or funder guidelines are met. Assists in program development work. OTHER RESPONSIBILITIES: Attends staff trainings and meetings as required. Other related duties as assigned. EDUCATION/EXPERIENCE REQUIREMENTS: Bachelor’s degree in Human Services, Psychology, Sociology, or related field
or
equivalent education/experience. EDUCATION/EXPERIENCE PREFERRED: Case management experience in a social service setting, and a working knowledge of issues and resources in relation to housing services for low-income families preferred. Experience working with persons living with mental illness. Experience working with persons experiencing chronic homelessness. Experience working with persons experiencing problematic substance uses. Knowledge of the disease of Chemical Dependency, its signs, and symptoms. SPECIAL REQUIREMENTS: Valid Washington State Driver’s License by time of hire. Good driving record. Driving abstract must be submitted upon request. SKILLS AND ABILITIES REQUIRED: Verbal and written communication skills. Computer skills, which include word processing, data entry, e-mail, etc. Interpersonal skills and the ability to interact positively with clients from diverse populations. The ability to create an innovative approach or new procedure or system as appropriate. Ability to maintain accurate and timely case management records. Remote Work : Job is on site. No remote work. Essential Employee : No Working Conditions: Work is performed in a fast-paced office environment with frequent interruptions and occasional crisis situations or in the community. Regularly requires travel within the agency’s service area for job development, meeting with clients, community networking, meetings, special workshops and trainings, and other job-related activities.
