**Responsibilities**

Mayo Clinic Rochester is seeking a motivated and experienced Advanced Practice Provider (APP) to join our Cardiology team, focusing on Pulmonary Hypertension (PH) within the Circulatory Failure Division. The successful candidate will play a pivotal role optimizing cardiovascular outcomes for patients with PH across various etiologies, utilizing evidence-based care. This role includes pre-visit reviews, seeing new patients in a team approach with an MD, early outpatient follow-up of PH patients dismissing from the hospital and providing longitudinal care to established patients. Additionally, there will be numerous opportunities aligned with the Bold.Forward.Unbound strategies in the coming years, fostering growth and professional development within the practice.

Key Responsibilities:

· Manage and provide longitudinal care for patients with PH cross various etiologies, including pulmonary arterial hypertension (PAH), PH caused by left sided heart disease, chronic lung disease, and chronic thromboembolic PH.

· Assist in the comprehensive management and titration of pharmacological therapies for various PH etiologies. Tailor treatment plans based on the specific pathophysiology of each PH type to optimize patient outcomes

· Coordinate and assist with interpretation of invasive hemodynamic tests

· Collaborate with cardiologists, pulmonologists, and other healthcare professionals to deliver multidisciplinary care.

· Educate patients and families on disease management and treatment options.

**Qualifications**

Master's or DNP prepared graduate of an accredited school of nursing and Nurse Practitioner. Current Minnesota RN license and certification as an FNP or ANP.

Or graduate of an accredited Physician Assistant program with current PA certification by NCCPA and licensure as a PA by the Minnesota board of Medical practice.

Must be eligible for DEA registration and billing privileges.

Cardiology experience preferred, but not required. New grads will be considered.
