Required experience:
- 5+ years within a Skilled Nursing Facility
- Prior Director of Nursing experience, Regional experience preferred
Schedule: This role would have a schedule of 10 days on, 4 days off. Traveling would be to different Brookdale CCRC communities (currently located in 12 different states: Alabama, Arizona, California, Colorado, Florida, Illinois, Kansas, North Carolina, Ohio, Rhode Island, South Carolina, and Texas).
This position would fill in for different clinical leadership roles within the CCRC communities and supervise licensed and non-licensed direct care associates. Responsibilities:
- Manages the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met.
- Ensures residents are treated with respect and dignity and ensures quality care as residents’ healthcare needs change.
- Supervises licensed nurses and other direct care staff within the community.
- Responsible for overseeing and validating all resident care to ensure needs are met and that care is provided in accordance with physician orders, care plans, policies/procedures, and state and federal regulations.
- Strengthens clinical processes within the community until a replacement is identified, then supports newly assigned Director of Clinical Services, Assistant Director of Clinical Services, and/or MDS Coordinator and others as assigned.
- Travels within the division as assigned to support management vacancies, start-ups, vacations, etc.
- Responsible for the direct supervision of community-based licensed nursing staff (LPN/LVN, RN), CNAs, and Med Techs/CMAs (if allowed/required by State Regulations). Also responsible to supervise nursing administrative associates.
- Assigns and directs work of subordinates, appraises performance, rewards and disciplines associates, and addresses complaints and resolves problems.
- Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act.
- Provides training and education to resident care associates on an ongoing basis with classroom in-services, situation-specific training, and resident-specific training.
- Supervises the maintenance of resident charts and reviews documentation performed by caregiving staff.
- As described and allowed in the Nurse Practice Act, assesses the health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness, and maximizes the resident's opportunity to remain in their environment.
- Manages the health care of residents, including the dissemination of information to families and associates.
- Participates in pre-admission screening of prospective new residents.
- Ensures that required documentation is completed prior to or upon resident admission.
- Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to ensure quality, proactive care.
- Evaluates residents; documents changes in condition, and notifies executive director, physician, and resident’s legally responsible party/family of resident's condition and reactions.
- Facilitates continuity of care for residents receiving home health care, hospice services, and other third-party healthcare-related services.
- Participates in or leads meetings relevant to resident care issues.
- Ensures in-house ancillary medical services are scheduled and followed through.
- Participates in department quality improvement activities.
- Assists in keeping the environment safe for associates.
- Shares on-call and manager-on-duty responsibilities/shifts as required.
- Strengthens clinical process within the community until permanent replacement is identified.
- Supports other requirements within the division as assigned.

Qualifications:
Education as required to obtain state nursing license (RN). Five years of skilled nursing experience as a Director of Nursing (DON) or District/Regional Nurse Consultant required. Certification in Infection Prevention/Control and RAI process preferred. Basic typing skills, along with a working knowledge of personal computing and word processing software, are required, preferably in a Microsoft Windows environment.
Certifications, Licenses, and Other Special Requirements: RN license.
