About the role and where you will be working:

This role provides guidance to the Wellness Partners and Service Coordinators. To work collaboratively with management, employees and clients to enable the delivery of high quality service to improve clients health, wellbeing and independence. This position will oversee the team located North of the River.

This position is located in our central office in Rivervale, WA. Flexible working environment available.

We’ll also be asking you to:

  • Comply with relevant legislation and regulations, including but not limited to the Home Care guidelines and Standards identified in the Aged Care Act 1997.
  • Ensure policies, procedures and work systems used by the team are within best practice.
  • Working knowledge of relevant industry legislation and compliance and frameworks as it applies to the role.
  • Work and maintain good working relationships with all stakeholders to ensure best practice outcomes for Home Care Clients.
  • Provide support to resolve client complaints and issues.
  • Demonstrate a commitment to quality improvement and pro-actively participate in quality improvement activities.
  • Ensure all clients have a current and high-quality Care Plan that meets or exceeds the requirements of the Aged Care Standards.
  • Undertake regular internal audits to ensure care and support is being provided within the limits of the funding and care is delivered in accordance with the Home Care Guidelines and Standards identified in the Aged Care Act 1997.

About you:

This person will have previous leadership experience in a health care setting, 5 years case management experience, and the ability to multi-task.
