Job Description
TOUR 2: 8am-4pm
AREAS OF RESPONSIBILITIES:
Inpatient Psychiatry
SUMMARY OF DUTIES AND RESPONSIBILITIES:
Under a collaborative agreement with an attending Psychiatrist and with the latitude for independent initiative and judgment, the Psychiatric Nurse Practitioner will perform the following functions: • Provides professional/clinical services to patients in inpatient Psychiatry to assess and address the need for psychiatric and/or substance abuse treatment, psychiatric medication(s), screening and referral for medical care, and follow-up as appropriate. • Serves as the treatment team leader, manages the care of selected patients within established protocols in the inpatient Psychiatry Service. • Leads and coordinates overall treatment of patients and provides treatment, including but not limited to psychopharmacology, psycho-education and supportive psychotherapy, as needed. • Performs a comprehensive psychiatric evaluation that includes assessing current psychiatric symptoms and past psychiatric history, the psychosocial and physical health status, evaluation of mental status, current and past history of violence, suicidal or self-harm behavior, substance use, level of functioning, health behaviors, trauma, and social and developmental history. • Completes initial Psychiatric Assessment, Suicide Risk Assessment, AIMS, and appropriate rating scales on the first treatment day with patient. • Reviews the medical record if the patient has had prior hospitalizations • Contacts the patient's outpatient psychiatrist (or clinician) to exchange clinical information if patient has been treated outside of hospital. • Documents patient's clinical and treatment status daily for first five working days and a minimum of three times a week thereafter. There must also be a note when there is a significant change in the patient's condition or treatment. • For patient on alternate level of care (ALOC), document a minimum of one note a week and whenever there is a significant change in the patient's condition or treatment. • Documents any psychiatric, behavioral, substance abuse and active medical problems on the • Problem List and Comprehensive Treatment Plan within three days of admission. • Develops a differential diagnosis derived from the collection and synthesis of assessment data. • Consults with other members of the interdisciplinary team to determine, evaluate, and plan • treatment goals/objectives, initiates and completes the Comprehensive Treatment Plan (along with the social worker and nurse) within three days of admission, • Updates and completes the Treatment Plan Review weekly after the completion of the • Comprehensive Treatment Plan and/or whenever there is a significant change in the patient's • condition or treatment. • Records patient's status and progress of care rendered, and plan for future care and therapeutic interventions appropriate to the patient's needs. • Counsels and guides patients, their families, and/or significant others relating to physical • health, psychiatric treatment, and progress, • Request appropriate laboratory tests within established protocols, and interprets findings of • relevant diagnostic and laboratory tests. • Prescribes and makes adjustment on medication within established protocols, and follow • such medication(s) interventions as appropriate' • Manages psychiatric emergencies by determining the level of risk and initiating and coordinating effective emergency care. • Serves as team leader in psychiatric crisis situations. • Ensures that patient and staff debriefings are conducted immediately following Code Greys or • other behavioral crises, leading the debriefings if possible' • Ensures that a patient Behavioral/Safety Plan is in place for all patients, and that team members are aware of and consider patient preferences in responding to a crisis. • Consults with the physician, as appropriate, for evaluation of findings of the psychiatric and • health screening in order to make prospective decisions about treatment. • Ensures the timely and appropriate management of a patient's medical problems by the unit's internist. • Leads medication education group for patients as directed. • Writes discharges prescription orders at least on day in advance of the discharge date. • Completes Discharge Summary (along with the attending psychiatrist, social worker, and nurse) and a discharge note within one day of discharge. • Ensures that patient is aware of discharge 24 hours before it occurs. • Works with service line supervisors to address performance issues with team members on an • ongoing basis and may provide input to service line supervisor on team member performance • evaluations. • Monitors unit specifics metrics, such as LOS, 30-day readmissions, discharges, etc. • Reports on unit's problems and significant administrative and clinical issues in the Departmental Morning Rounds. • Notifies the Unit Chief and/or Director of inpatient Psychiatry of sentinel events, incidents, Code Greys, elopements, deviations from standards of cares and problems that may affect the customary and usual activities of the unit. • Conducts investigation of incidents occurring on the unit. • Makes appropriate referrals to other health care professionals'
Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and 2. Current certificate as a Nurse Practitioner issued by the NYSED; and 3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Bachelors of Science in Nursing (BSN) or Bachelors in allied health field = $900 Masters of Science in Nursing (MSN) or Masters in allied health field = $1,200 Practice or PhD in Nursing (DNP) = $2,500
An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the field or area of specialization for which you are employed. An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
