APRN, Per Diem -Healthcare for the Homeless

Provides advanced nursing functions within the scope of the Nursing Practice Act (NH) to client/patients of all ages and their families in the Mobile Community Health Team (MCHT)/Health Care for the Homeless Program (HCH).

Primary Functions

Family practice preferred. Individuals must possess the following knowledge, skills and abilities or be able to explain and demonstrate that the individual can perform the essential functions of the job, with or without reasonable accommodation, using some other combination of skills and abilities and to possess the necessary physical requirements, with or without the aid of mechanical devices to safely perform the essential functions of the job.

• Ability to deliver patient care in a manner that is appropriate to the demographics and/ or specific needs of the patient population served.

• Demonstrates proficiency in assessing, responding and adapting treatment of care to meet the needs of the patient population served.

• Ability to transport self to and from off-site locations.

• Ability to exert up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or constantly to lift, carry, push, pull or otherwise move objects.

• Ability to demonstrate courtesy, compassion and respect in relating to client/patients, families, visitors, physicians, and co-workers.

• Ability to communicate effectively and establish a cooperative, collaborative working environment.

• Ability to facilitate communication between and among others.

• Ability to accept constructive feedback and initiate appropriate actions to correct situations.

• Willingness to identify and take action toward meeting own learning needs.

• Ability to endure periods of heavy workload or stress.

• Ability to maintain good communications; ability to establish and maintain positive working relationships with employees, physicians, client/patients, families, and the public.

Qualifications

Education

• Graduate of an accredited APRN program.

Experience

• Two (2) years of experience as mid-level provider

• Experience or interest in caring for people who are homeless or in poverty medicine population settings, preferred

Licensure/Certification:

• Advanced Cardiovascular Life Support (ACLS) certification preferred.

• Licensed by the state of NH as an Adult practice APRN
