Faith Presbyterian HospiceHospice Registered Nurse - Inpatient Unit7pm - 7amResponsibilities:ESSENTIAL DUTIES:Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workersAssume primary responsibility for a patient admission including assessment if needed, calls report of admission to teamActively participate in coordination of all aspects of patients care, in accordance with current professional standards and practiceInitiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/familyPractices Universal Precautions according to OSHA regulations and Faith Hospice policyDevelop and evaluate the plan of careParticipate in QAPI program and hospice sponsored in-service trainingParticipate in ongoing interdisciplinary comprehensive assessmentMaintains confidentiality of all patient information, sharing only what needs to be shared with the interdisciplinary team in order for the patient to be adequately cared forMaintain regular communication with the Director of Business Development, Director of Clinical Operations and Patient Care Manager in order to follow-up for every admission.Respects patient/family rights by: (knocking on doors before entering the room, addresses patient/family by name, allows patient to make own decisions when appropriate)Provides emotional support and compassionate care to the patient and family. Demonstrates listening skills, offers encouragement when appropriate, adjusts care to the patients physical condition each visitObtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.Instruct/Educate primary caregivers, volunteers, and employed caregivers to provide care as indicated; Contribute to patient and family counseling and educationCompletes admission process and nursing assessment and reportsInforms the Patient Care Manager of unusual or potentially problematic patient/family issues.Assist other nurses including assuming primary responsibility for patients when necessaryAttends and participates in orientation and appropriate in-services and staff meetings to improve knowledge and skills and to maintain competency.Provide appropriate support at time of death. Reports death to appropriate staff.Plans the administration of and/or administers medication with knowledge of actions, contraindications, side effects and desired outcomes.Participates in agency and community programs as requestedIs available for holiday, on-call, illness and vacation coverage as needed.Other duties as assigned by Patient Care Manager.Qualifications:MINIMUM QUALIFICATIONS:Minimum of two years of varied work experience as a professional nurse and preferably one of the two years in an hospice inpatient settingPossession of a valid Texas Registered Nurse LicenseProficient ability to communicate orally and in writing in the English languageValid Texas Drivers License and reliable method of transportationAbility to work independentlyAbility to adjust to changing conditions and frequent demandsAbility to acquire new knowledge to perform additional or more difficult tasksAbility to maintain a positive, stable disposition in performing, working in association with staff, patients and familiesAbility to maintain a high level of performance in an environment of frequent demands and changesAbility to meet new and/or stressful situations with the appropriate responses, ability to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal and written instructionsAbility to perform Essential Duties as outlined belowAbility to perform work tasks within the Physical Demand Requirements as outlined belowGenuine caring for and interest in the care of mature adults, handicapped people and hospice PatientsAbility to perform work tasks within the Physical Demand Requirements as outlined belowAbility to comply with the Dying Persons of Rights and the Employee ResponsibilitiesAbility to comply with organizational and departmental safety policies and procedures
