The clinical teams at the areas clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being.GeneralEstablished team of 2 physicians and 1 Advanced PractitionersEstablished practice serving the Dubuque communityExperienced staff, both clinical and non-clinical, who know the patients and the communityFull-time position, including dedicated administrative time, with flexible scheduling to ensure sufficient patient access24/7 hospitalist program is responsible for adult and pediatric inpatientsFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationResponsibilities:
Average of 18-20 patients per dayShared call amongst the providers. Currently 1:3 night call taken day-to-day; Physician sees own patients for call during business hoursPractice is a mix of inpatient and outpatient; 2-4 interventions per day when in clinicAbility to work as a team member to improve the health of the people and communities we serveTwo nearby outreach locations are visited by Physician once per monthEvaluates, interprets and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical and problem-solving skillsQualifications:
