The clinical teams at the areas clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being.Established team of 2 Physicians, 1 PRN Physician and 1 Advanced PractitionerOpportunity to practice outpatient medicine in an established and well-known clinicCohesive group of colleagues and supportive staff allowing for autonomy while building the practice according to your preferencesExperienced staff, both clinical and non-clinical who know the patients and the communityFull-time position including dedicated administrative time, with flexible scheduling to ensure sufficient patient accessFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationResponsibilities:
Average of 18-20 patients per day; no nights or weekends, 4.5 day work weekDedicated roomer, lab person in clinic, care manager and care coordinator, clinic attached to hospitalAfter hours call is shared, every other weekend, very light call - triaged through nurse lineAbility to work as a team member to improve the health of the people and communities we serveEvaluates, interprets, and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical, and problem-solving skillsQualifications:
