Job Title: Care Manager - Non-Medical Senior CareCoast Care is one of the largest Home Care Organizations in San Diego. We are locally owned with over 200 years of combined professional experience in the industry and are adding an experienced Care Manager to our team. In this role you will get to make a meaningful and positive impact to the lives of our clients, caregivers, and organization and enable the compassionate and empathetic delivery of essential home care services.Job Summary:The Care Manager in Non-Medical Senior Care is responsible for overseeing the well-being of seniors, ensuring the highest quality of care, and managing the retention of clients. This position requires a nursing background, LVN or RN preferred. The Care Manager will play a key role in rightsizing services, providing care oversight, and effectively managing caregivers to enhance the overall experience for seniors and their families.Responsibilities:Client Retention:
Develop and maintain positive relationships with clients and their families to ensure satisfactionConduct regular check-ins and assessments to address client needs, preferences, and concerns
Rightsizing Service:
Collaborate with clients and families to assess and adjust care plans based on changing needs and preferences.Ensure that services provided align with clients' evolving requirements, promoting a personalized and adaptable care approach.
Care Oversight:
Conduct comprehensive assessments of seniors' physical, emotional, and social needs.Develop and oversee individualized care plans, ensuring they align with best practices and clients' preferences.
Caregiver Management:
Train and supervise caregivers, ensuring they provide high-quality, compassionate care.Conduct regular evaluations of caregiver performance and provide constructive feedback.Address caregiver concerns and provide support as needed.
Advocacy:
Advocate for the rights and preferences of seniors in care decision-making.Work closely with clients, families, and caregivers to address concerns and ensure open communication.
Documentation:
Maintain accurate and up-to-date records of client care plans, assessments, and caregiver assignments.Document and communicate any changes in client status or care needs.
Quality Assurance:
Participate in ongoing quality improvement initiatives to enhance the overall quality of care.
Collaboration:
Collaborate with interdisciplinary teams, including healthcare professionals, to optimize client care outcomes.Participate in regular team meetings to discuss client cases, share insights, and address challenges.
Qualifications:RN, LVN or equivalent experienceProven experience as a Care Manager or similar roleOutstanding communication skills, both written and verbalAbility to travel locally between clients homesCoast Care Partners is an equal opportunity employer.
