Job DescriptionWith the program planning team and consumer, develop, implement and monitor the residential habilitation plan and IPOPImplement the Individualized Service Plan (ISP) as developed for each consumer by his/her family advocate and Service CoordinatorEnsure that the consumer's medical needs are met by ensuring the following: medical appointments are scheduled, making arrangements to ensure that the consumer gets to the appointments, communicating all necessary information to nursing staff, the Service Coordinator, family advocate, residential staff and scheduling follow up or referral appointment as neededCoordinate transportation, as needed, for consumersResponsible for ensuring that the consumer's financial matters are handled within agency procedures, including management of the personal needsResponsible for the hiring, orientation and ongoing training, supervision and evaluation of all direct care staff, including the site supervisor assistantWorks with the community services nurse to supervise and evaluate the performance of all medication administration certified staff, including developing and monitoring corrective action plans as needed, in conformance with agency policy and proceduresResponsible for the scheduling of direct care staff ensuring that the number and types of staff assigned are adequate to meet the needs of the consumersWith the site supervisor assistant responsible for ensuring that there is a supervisor available 7 days a week during key program hours (weeknights, holidays and weekends)Become certified in medication administration and maintain certification on an annual basisProvide direct care services, as neededComplete monthly paperwork accurately (i.e. attendance records, petty cash, med fund, gas fund, monthly receipts, personal allowance records, etc.)Ensure residential habilitation services are documented in accordance with Agency, OMRDD and Medicaid requirementsEnsure that the agency vehicles are maintained and required transportation paperwork is complete, per policyPrepare and submit management reports, as requested/neededResponsible for ensuring site is managed within budgetResponsible for upkeep of the residence, including requisitioning of maintenance for repairs to certified homesParticipate in on-call rotationResponsible for ensuring that there are adequate and appropriate recreational activities in the community scheduled for consumers year roundResponsible for overseeing that the nutritional needs of the consumers are met and that there are balanced and healthy menus prepared or available for consumers who prepare their own mealsParticipates in corporate compliance and quality assurance activities as requiredParticipates in quality improvement and improvements, as well as corporate compliance activity as needed.Demonstrates adherence to the DSP Code of Ethics.Support the Brand Platform of the Agency by incorporating the brand personalities Competent, Dedicated, Professional, Caring and Innovative - throughout your daily work.Other related duties as assigned.

Qualifications/Experience Required-Minimum of a high school diploma or equivalent with 5 years' experience working with developmentally disabled adults, OR-Associate's Degree in Human Services or related field and at least two years experience with developmentally disabled adults, OR-Bachelor's Degree in Human Services or related field with at least one year experience providing service to people with developmental disabilities is preferred.-LPN or RN license may be substituted for Associates Degree.-A minimum of one year supervisory experience, with excellent supervision skills, is required-Valid NYS drivers license with a safe driving record per agency policy: use of personal vehicle is required
