What you'll do:
Evaluate the skilled physical therapy needs of referred patients, identifying problems, and developing comprehensive plans of care.Communicate effectively with patients, ensuring their understanding and agreement to proceed with the treatment plan. Also, provide instruction to patients and their families on the use of home medical equipment and home exercise programs.Direct and assist patients in therapeutic interventions focusing on various aspects of physical well-being.Document all assessments in the medical record in a clear, concise manner, adhering to timelines for evaluations, re-evaluations, and discharge summaries.Demonstrate discharge planning skills and actively participate in various meetings, including family, team, and case management meetings.What you'll bring:
Must be a graduate of an approved Physical Therapy Program with a Bachelor's, Master's, or Doctorate of Physical Therapy Degree.
New graduates are considered.Professional memberships and development of career goals are encouraged.
Must be licensed in the State of New Hampshire as a Physical Therapist by the NH Office of Licensed Allied Health Professionals.American Heart Association Basic Life Support (BLS) certification required.Proficiency in assessing, responding, and adapting treatment to meet the needs of the patient population.Knowledge of APTA Code of Ethics and state licensure law and practice act.
