This role offers a unique opportunity to provide high-quality patient care through both in-person visits and advanced telehealth and remote patient monitoring. The ideal candidate will excel in blending clinical expertise with cutting-edge technology to deliver comprehensive, patient-centered care. A passion for leveraging digital tools to proactively manage patient health, improve outcomes, and enhance the patient experience is essential.DESCRIPTION:Duties and Responsibilities:Perform comprehensive patient assessments, establish diagnoses, develop treatment plans, and manage medications.Utilize telehealth platforms and remote monitoring devices to conduct virtual consultations and oversee patient health remotely.Analyze data from remote monitoring devices to identify trends, detect abnormalities, and recognize opportunities for intervention.Collaborate closely with physicians and interdisciplinary team members to create and implement personalized care plans that integrate both in-person and remote care strategies.Educate patients on the use of remote monitoring devices, medication adherence, lifestyle modifications, and self-management strategies to promote wellness and prevent complications.Communicate effectively with patients via telehealth platforms, offering continuous support, answering questions, and addressing concerns.Maintain accurate and comprehensive documentation of all patient interactions, assessments, and interventions in electronic health records.QUALIFICATIONS:

Required Knowledge, Skills, and Abilities:Graduate of an accredited Nurse Practitioner program.Current Illinois licensure as a Nurse Practitioner.Certification by a recognized Nurse Practitioner certifying body (e.g., ANCC, AANP).Demonstrated experience or strong interest in telehealth, remote patient monitoring, and digital health technologies.Exceptional communication and interpersonal skills, with the ability to engage patients effectively both in-person and remotely.Strong clinical assessment and critical thinking abilities, with the adaptability to meet evolving patient needs and integrate new technology solutions.Proficiency in telehealth platforms, electronic health record (EHR) systems, and remote monitoring devices.Ability to work both autonomously and collaboratively within a remote team environment.Commitment to delivering patient-centered care, promoting health equity, and optimizing patient outcomes.Preferred Qualifications:

Previous experience as a Nurse Practitioner in primary care, urgent care, or a related clinical setting.Knowledge of chronic disease management, preventive care strategies, and population health principles.Familiarity with remote monitoring devices, wearable health technologies, and data analytics tools.Bilingual proficiency.Application Process:
