PRIMARY PURPOSE:

Serves as a primary counselor for patients in the Outpatient Treatment Program. Provides support, supervision, crisis intervention substance abuse and psychosocial rehabilitation services to patients with a mental health and substance use disorder. The Primary Counselor is responsible for planning, organizing and implementing a variety of groups and weekly individual sessions. The Primary Counselor is responsible for management of a recovery environment to include implementation of individual recovery plans.ESSENTIAL FUNCTIONS:Ability to interact and work with patients, co-workers, and supervisor in a pleasant manner at all timesPatient caseload (10-12 Day tx and IOP) as assigned by OP treatment managerEfficient use of time, ability to meet deadlinesCompletion of all clinical paperwork within the time frames for assigned patientsResponsible to call Case Manager, Probation Officer, Forensic Coordinator, and all pertinent care team members during the individual recovery session to report progress or lack of progress. Documenting interactions in the patient NAP notes using WITS the same day.Preparing and providing a variety of education, task, and or process groups as specified on the weekly schedule, e.g. mental health, dual diagnosis, relapse prevention, seeking safety, motivational interviewing, anger management, 12-step recovery, vocational rehab, smoking cessation, etc.Participates as part of the recovery team in patient evaluation and recovery as well as the individual recovery planning.Assessing and evaluating mental health and substance abuse recovery.Provide support which helps patients develop skills and supports to control symptoms.Instructs and assists patients with communication and stress management skills.Maintaining current and proper authorizations for all funding sources on an on-going basis and pay attention to funding source changesCompletes the transition and discharge status changes into the WITS programMaintaining proper consent forms on an on-going basisQUALIFICATION REQUIREMENTS/SKILLS/KNOWLEDGE/ABILITIES:Ability to write patient progress letters to various departments, i.e. parole, probation, Forensic Coordinator, psychiatrist, etc.Computer literate (Word) – must know how to save documents properlyAbility to work in a fast-paced, high-stress, environment that requires rapid decision makingOTHER REQUIREMENTS:Current CPR and First Aid Certification.Current TB Clearance (annual, or annual clearance for negative Chest-x-rays)Current Physical Clearance (annual)Current Criminal Abstract (annual)Current, Clean Drivers Abstract (annual)Maintain confidentiality of all patient records at all times.
