Conduct outpatient appointments in the clinic setting and participate in hospital rounds to monitor and manage hospitalized patientsCollaborate with interdisciplinary teams to coordinate and ensure seamless continuity of care for patientsProvide education and counseling to patients and their families regarding cardiovascular health, treatment options, and lifestyle modificationsAbility to work as a team member to improve the health of the people and communities we serveEvaluates, interprets and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical and problem-solving skillsQualifications:
