We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team as the Integrated Clinical Services.The Immediate Access Provider will provide same day access to new and established patients of Integrated Clinical Services. This position will reduce referrals to external Urgent Cares and Emergency Departments.

The

Immediate Access provider

does not have a patient panel, but is an integral part of the care team to ensure care continuity in the primary care setting. The Immediate Access provider will have 0.8 clinical FTE with same day and next day availability, and 0.2 FTE for indirect patient care time.

As a newly hired

Advanced Practice Clinician

you will receive a signing bonus of $2,500 prorated based on FTE (0.8 FTE and higher will not be prorated). This bonus will be paid in one lump sum on the next regularly scheduled pay date after you start employment with Multnomah County. Furthermore, there will be a retention bonus of $2,000 prorated based on FTE (0.8 FTE and higher will not be prorated) after one year of continuous regular employment. This signing bonus is taxable, and all regular payroll taxes will be withheld.

As the

Immediate Access Provider other duties include:

Clinical Care DirectPerform comprehensive health assessments and clinical procedures by completing a physical and psychosocial history; identify actual and potential health needs based on medical, behavioral, financial, and social factors; prescribe and dispense medications to patients; initiate and/or continue medical regimens for new and established patients.Examine, diagnose, treat, and advise patients on medical and behavioral health problems; make referrals to appropriate social or medical agencies; chart treatment records and monitor results; provide patient education and institute treatment plans.Perform treatments such as suturing simple lacerations, performing routine excisions and biopsies, removing foreign bodies, conducting wound care and debridement, and casting of simple fractures;; interpret various diagnostic studies such as lab values, X-rays, EKGs, sonograms, and EEGs.Act as representative on committees, interagency task forces, special projects, and other community outreach activities as assigned; provide consultation and education to internal and external partners and providers.Analyze and review federal, state, and local laws, regulations, policies, and procedures in order to ensure compliance; conduct analysis on best practices and trends, and formulate recommendations.Clinical Care Indirect

Seeks clinical consultation from other clinicians as needed as part of practice management.Reviews tests, laboratories, and diagnostic imaging ordered in the course of care, and develops a care plan to ensure continuity with primary care.Provide consultation to other providers, nurses and support staff.Prioritize and direct activities of the care team.Coordinates with primary care provider behavioral health, pharmacy, and dental providers to effectively integrate care.Provides coverage for clinical colleagues in their absence.Completes patient documentation and manages electronic in-basket according to established policies and procedures.Follow-ups on utilization issues (ED, hospital admissions) including specialty referrals and consultations.Participates in Team, Professional, and Health Center-wide meetings.Participates in continuing education to maintain licensure, and current, evidence based practice.TO QUALIFY:

Required Minimum Qualifications/Transferable Skills*:

Masters degree or equivalent in nursing from an accredited college or universityPossession of a valid Nurse Practitioner license OR Possession of a valid Medical Doctor licenseCPR/BLS certificationDEA certificationPreferred Special Qualifications/Transferable Skills*:
