Position Overview
:
Reporting to our Clinical Manager, the Nurse Practitioner is an extension of the physician responsible for gathering clinical findings to help
determine
continuing eligibility for hospice patients.
The Nurse Practitioner covers PRN on-call emergency face-to-face visits with hospice patients and coordinates with the RN Case Manager, IDT Team, and sponsoring physician on all patient needs.
Key Responsibilities:
Collaborate with the IDT in assuring
appropriate evaluation
and palliative treatment of terminal disease.
Consult with and
assist
in the IDT’s development of patient/family plan of care to assure quality care is provided.
Consult with and educate the patient/family
regarding
disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
Perform and document visits to patient/family based on plan of care and patient/family need.
Interact with attending and consulting physicians as necessary
regarding
palliative care, in collaboration with hospice
medical director.
Consult with and provide education to other IDT members on a PRN basis for clinical issues relating to palliation.
Actively
participate
in the IDT.
Communicates effectively,
timely
, and appropriately.
Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
Complete face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
Provide clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
Adhere to agency policies
regarding
protected health information.
Maintain continuing
education/knowledge in pharmacology and palliative
medicine
Qualifications:
Graduate from an accredited
APN program.
Licensed as a Nurse Practitioner in state of practice
.
Minimum of 2
years’ experience
as
a
Nurse Practitioner
in hospice or home health.
Comprehensive knowledge of pharmacology, palliative care, and end-of-life
Current CPR certification.
