PathWell Health is a home health and hospice service provider in northern Virginia. We are seeking a dedicated and experienced Hospice Nurse Practitioner to join our team on a part-time, hourly basis. The ideal candidate will bring a strong background in hospice care, demonstrating expertise in participating in the Interdisciplinary Team (IDT), collaborating with medical professionals, and ensuring optimal patient care.
Responsibilities:
Order prescriptions as needed in consultation with the medical doctor, ensuring timely and accurate administration of medications.
Conduct Face-to-Face assessments if and when required, maintaining compliance with regulatory requirements
Actively engage in IDT meetings to contribute valuable insights and collaborate with interdisciplinary team members.
Demonstrate responsiveness and maintain on-call availability to order medications and address urgent patient needs promptly.
Liaise with our medical doctor and registered nurses to coordinate and optimize patient care plans.
Qualifications:
Current Nurse Practitioner license in the state of VA
Ability to work collaboratively within an interdisciplinary team.
Experience in hospice and palliative care.
Experience in ordering prescriptions and managing medication
Demonstrated ability to conduct Face-to-Face assessments
Commitment to maintaining the highest standards of patient care.
