The Nurse Practitioner provides quality hospice and palliative care to assigned patients across the life span (perinatal, pediatrics, and adults) while consulting with the team physician, associate medical director and chief medical officer (CMO). Consults, assesses, and evaluates the plan of care while collaborating with the IDT (interdisciplinary team), patient, family, and community physicians. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice.The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.Salary: $74 - $82 hourly. Subject to experience and qualifications.Work Schedule: Varies, 8-hour shifts. Candidate must provide availability for 6 shifts per month, 2 of which must be a weekend day.Work Location: San Diego County, CATravel: Travel is required for patient visits.Responsibilities

Responsible for visiting patients within a short period of time following admission and as medically necessary afterwards.Ensure documentation is entered into the electronic record within expected guidelines.Coordinates care with the interdisciplinary Team (IDT).Communicates all changes to IDT and community physician(s).Seeks medical direction for assigned patients as required including patients seen in the hospital.Utilizes critical thinking to interpret multiple, sometimes conflicting patient data.Makes clinical judgments based upon a grasp of the "whole picture" for patient diagnoses, managing changes of condition with appropriate levels of care.Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.Orients, instructs, and trains assigned personnel.Works with Education Program Manager to educate staff regarding patient care.When necessary, share in providing 24 hours, seven days a week coverage to patients and families.Available rotating weekends to make patient visits as needed.Attends medical staff meetings, nurse practitioner meetings and other meetings as directed by the CMO.Consults the CMO when a plan of care is outside of standardized practice and protocols, and specialty consults are needed to manage patients care.Counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.Documents comprehensive medical history in patient's electronic record. Identify pertinent patient information, progress, medications, and diagnostic studies as required. Keeps the interdisciplinary team updated on changes made to patients plan of care (per agency documentation standards).Establishes, builds, and nurtures relationships with staff and community referral sources to facilitate program growth. Participates in community events as requested by the marketing team.Adhering to the E-way standards is a requirement in performing all job duties.Qualifications

Must be licensed as a Registered Nurse by the State of California.Must be licensed as a Nurse Practitioner by the State of California.Must have Drug Enforcement Administration (DEA) Certificate.Must have valid National Provider Identification (NPI) Number or must be obtained within two (2) months of hire date.Must have successfully completed a Nurse Practitioner (NP) Program from a recognized university and obtained a Master of Science Degree in Nursing (MSN).
