Come be a part of the Infant and Early Childhood team at Touchstone. This is a wonderful position for individuals who are passionate about early intervention and learning how to work with young children and families. This position provides effective middle to high level case management, clinical perspective, and service coordination for clients and their families in accordance with the Arizona Practice Model 12 Principles. They work collaboratively with our team members and support families in accessing resources within Touchstone and in the community.

Qualifications:• Must be at least 21 years of age• Bachelor's degree or High School Diploma and at least four (4) years of experience providing behavioral health services; or an acceptable and equivalent combination of education and experience• Must meet Agency requirements for Behavioral Health Technician• Must possess a valid AZ Drivers License• Must possess an AZ Fingerprint Clearance Card or be eligible for one

POPULATION SERVED: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the consumers served in the department:X Infant & Early Childhood (birth - 5 years)

Essential Functions:1. Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA).2. Facilitates CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines in order to create individualized service plans as identified by the CFT members.3. Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensure family participation that links them to natural or community-based supports.4. Coordinates and schedules CFT/Service Plan meetings with natural, formal supports, and system stakeholders5. Completes comprehensive screenings with families after intakes to develop rapport with families, identify significant needs, and make appropriate referrals. Submits appropriate referrals within the agency and externally in the community.6. Updates and maintains electronic medical records on each case.7. Completes strength assessments and crisis/safety plans with families according to RBHA and ADHS guidelines.8. Develops positive supportive relationships with consumers, family, stakeholders, and other TBH employees.9. Completes additional duties as needed.
