Pay range per hour for PHN II is $44.34 - $58.15, and PHN III range is $47.40 - $60.84.Working with the Public Health Nurse Supervisor and under the direction of the Community Health Services Manager, the Public Health Nurse (PHN) I, II, or III performs general public health duties, including maternal child health (MCH) home visiting under the Growing Families program, providing immunizations, conducting general communicable disease case/contact investigations, and/or other duties as assigned within the PHN scope of practice. The PHN provides immunizations to high-risk populations and in outbreak situations, and participates in disease outbreak investigations and control, which may include medical countermeasures. The PHN partners with hospitals, health care providers, schools, other public health professionals, and populations at-risk to provide education and outreach on public health topics. This position will also provide public health nursing services to individuals, families, groups, and the community, including all socioeconomic levels and in largely unsupervised settings.Responsibilities

Perform outreach, education, and referral to individuals and populations as necessary for public health purposes.Provide health education for schools, childcare facilities, and other community groups on general public health topics and program-specific topics as needed. Participate in community events that will serve populations at risk.Partner with health care providers, clinics, and hospitals to educate on public health topics as needed. Outreach to providers, clinics, and hospitals to foster Growing Families referrals and partnership, as directed by the Lead Public Health Nurse and the Community Health Services Manager.Participate in state and local public health meetings to further work in public health program areas, as necessary.Work with public health emergency preparedness and response coordinator to plan and train for a variety of public health emergencies and respond to emergencies according to plans.Maintain confidentiality of patient information, records, and data pursuant to county policies and procedures, Washington State and federal HIPPA regulations.Acting within the scope of their responsibilities, work as a public employee and municipal officer with courtesy and professionalism, adhering to the highest standard of ethics in accordance with RCW 42.52.Perform other related duties as assigned.Public Health Nurse II (Program Coordinator)

All duties above and the following:Provide assigned services and program work across all islands in San Juan County. This may involve weekly travel to other islands to provide services to all community members and travel for program work outside of the individual’s assigned program.Develop relationships with community partners and agencies related to the assigned program areas.As assigned, the PHN II will orient and cross-train other PHNs to program work in areas for which they are program coordinator in order to provide for service continuity and time-off as needed.The PHN II will serve as a subject matter expert for their assigned program(s) and work with the Community Health Services Manager and the Public Health Nurse Supervisor to ensure proficiency and determine professional development plans and opportunities.The PHN II will coordinate meetings and communications between staff completing work for the program for which they are the coordinator.The PHN II will take initiative for managing supplies for their assigned program’s operations across all islands and in the mobile clinic van, as appropriate.Public Health Nurse III (Program Lead)

All duties from PHN I, PHN II, and the following:The PHN III will orient and cross-train other PHNs to program work in areas for which they are program lead in order to provide for service continuity and time-off as needed. The PHN III will work with the Community Health Services Manager and the Public Health Nurse Supervisor to develop and implement a cross-training plan for this purpose.The PHN III is responsible for completing any reports as required for program implementation and/or funding.The PHN III will lead the program(s) to which they are assigned, ensuring service provision and continuity for the community receiving services.Minimum Requirements

A combination of education and related experience sufficient to provide the requisite knowledge, skills and abilities. A Bachelor’s Degree in Nursing from an accredited school of nursing and a valid license to practice as a Registered Professional Nurse in the State of Washington, OR any equivalent combination of education and experience that demonstrates the ability to perform the essential functions of the job. Requires a valid Washington State Driver’s License. Required to complete and pass background check per Department criteria.PHN II

Employees within this range have a minimum of two years of experience in public health nursing, or clinical nursing with a working knowledge of public health principles, nursing protocols, and public outreach and education. This is the PHN program coordinator position.PHN III

Employees within this range have a minimum of four years of experience in public health nursing, or clinical nursing with a working knowledge of public health principles, nursing protocols, and public outreach and education. This is the PHN program lead position.PREFERRED QUALIFICATIONS

Bilingual English/SpanishAdvanced degree in Public Health NursingPublic Health Nurse certification or trainingCertified in Public Health (CPH)KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of motivational interviewing.Knowledge of the principles of communicable disease investigation, prevention, and control measures in a community setting.Requires ability to translate complex medical concepts into clear and understandable language.Must have respect for and ability to work with people of diverse ages, abilities, and socioeconomic status in a culturally relevant manner.Requires ability to work with the public in potentially confrontational or adversarial situations.Ability to communicate effectively in both oral and written format.Ability to work independently, with good judgment and a minimum of supervision.Ability to plan, organize, prioritize and coordinate work assignments and/or projects, meeting required deadlines.Ability to formulate solutions to problems in a timely manner.Ability to establish and maintain effective working relationships with co-workers, community agencies, and the general public using courtesy, tact, and good judgment.Demonstrated ability to maintain a high level of confidentiality.Demonstrated ability to work with computers and computer software programs including email, databases, word processing and spreadsheets.Ability and skill to utilize motivational interviewing with successful outcomes.Ability to conduct CD investigation, prevention, and control measures in a community setting.Knowledge of and ability to interpret/apply relevant local, state, and federal nursing policies, procedures, protocols, and guidelines.PHYSICAL AND MENTAL REQUIREMENTS

Requires frequent sitting, standing and walking.The employee is frequently required to lift objects weighing up to 25 pounds and must infrequently lift more than 50 pounds or carry objects with both hands and arms.This position frequently requires continuous hand and finger movement.This position frequently requires visual acuity at 20 inches or less, and frequent hearing acuity for speech discrimination.The employee must be able to talk and to receive and understand written and oral communication and give written and oral instruction.The position requires the ability to occasionally calculate mathematical problems.Must receive preventive immunizations against common communicable diseases in San Juan County as recommended for health care workers by the Advisory Committee of Immunization Practice (ACIP).Work requires operation of a computer including keyboard and mouse operation.Involves talking and hearing in person and on the phone.Vision abilities required by this job include close vision and the ability to differentiate colors and read computer monitor.Deals with customer stress and deadlines, which may require overtime, flexibility of time.This position requires attention to detail and communication skills.WORKING CONDITIONS

Works in a variety of settings, including home, office, schools, business sites and other community settings. Physical setting may be unsanitary and potentially dangerous. Encounters occasional traumatic results that require post-event counseling. Works occasional weekend and off hour calls. Travel less than 10% of the time. Possible exposure to blood borne pathogens and other communicable diseases. Potential for exposure to hostile individuals and settings. Requires the ability to maintain maximum flexibility regarding schedules and workloads. Some travel to other locations may be required by boat or by air, including for field operations, which may involve inclement weather conditions.
