RESPONSIBILITIES:
Provides outreach to engage high-risk youth in services.
Meets with parents and guardians to assist engagement
Provides counseling, employment, educational, and life skills training
Coordinates with other YOS services to engage youth and families in appropriate services
Provides transportation to youth as necessary
Maintains contact or establishes relationships with referral sources beneficial to the client and/or agency
Makes referrals to appropriate programs and/or services both internally and externally and formulates necessary follow-up in a timely manner.
Refers youth to services as needed
Works with youth and other collaterals to develop service plans that assist youth in addressing challenges
Documents all interactions in designated electronic record
Participates in team/individual supervision and meetings as necessary
Collaborates with co-workers and participates in internal agency programs to provide the most comprehensive client services
QUALIFICATIONS:
BA in Social Work, Counseling, or Psychology is required
Experience working with at-risk youth and families
Experience with outreach programs
A valid driver's license
Reliable transportation
(proof of insurance is required)
