Pierce County's First Certified Community Behavioral Health Clinic: Comprehensive

Life Resources is looking for a qualified Registered Nurse to join our PACT team in Tacoma, WA.

This is an evidenced based practice to provide wrap around support to individuals struggling with serious mental illness. This exciting position would be part of a multi-disciplinary team consisting of mental health professionals, psychiatrists, nurses, peers, and case managers. This is an evidence-based practice that all team members will be trained to implement.

Responsibilities :Takes the lead of filling prescription orders, storing and putting together medication deliveries and packets, and ensuring that the Medication Administration Record (MAR) and all other documentation related to medications is accurate and up to dateManages IM injection schedule and administers injections both in the community, and in the officeConducts regular screenings for medical conditions, medication side effects (i.e. conducting AIMS), and monitoring existing medical conditionsHas regular contact with inpatient and outpatient medical and psychiatric care providers for clients in order to coordinate care.May take clients to doctor appointments, especially when the client's care is complicated, to ensure the best client careAssesses for risky behaviors (i.e. unsafe sex, not using clean needles) and work with clients on behavior change and/or ways to reduce the potential for harmAssesses and intervenes on health/medical risk factors or conditions by providing services such as education to clients and natural supports, and monitoring diets specific to diabetic needs, or to prevent and address hypertension and high cholesterolProvides cross training to other team members to help them monitor psychiatric symptoms and medication side effectsHelps clients develop strategies to maximize the taking of medications as prescribedTracks age related and family history health screens (i.e. mammograms for women over 40, prostate exam for men at age 50)Engages in strategies to reduce tobacco useAttends and participate in daily team meetingsQualifications :

Must be a Registered Nurse (RN) in WA stateExperience working with adults with severe mental illness preferredFamiliarity with Motivational Interviewing (MI) preferred, but not requiredDemonstrated ability to multi-task, prioritize deadlines, work independently, take initiative, and maintain confidentiality.
