Position Overview:

The salary min $90,000 to max $100,000.SCOPE OF ROLE:The Registered Nurse will work on OMH care team/s with mental health professionals assisting clients in transition from hospitals back to the community. The team will follow individuals into the community using the evidence-based Critical Time Intervention model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as medical case management, referrals, health teaching (this might include group education) and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require limited on call coverage.ESSENTIAL DUTIES & RESPONSIBILITIES:Complete medical assessments and Communicable Disease Risk Assessments within mandated time framesCoordinate delivery and reception of medications with pharmacyTrack all medications received and administered as well as coordinate return of unused medications to pharmacyObserve self-administration of medications for detoxification according to established protocolsRefer for required medical servicesCarry out urine screenings in accordance with program policyProvide education and consultation to individual clients and/or groups on their health needsAssess clients medical record and coordinate medical care with medical director and/or outside clinicsAssure clients compliance with prescribe medical regimenComplete timely and accurate documentation on medical services renderedKeep supervisor informed of all relevant information regarding the residents and the programOther duties as requested by supervisorBenefits Overview:We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance; 403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and HolidaysQualifications:REQUIRED EDUCATION AND EXPERIENCENYS RN licenseKnowledge of chemical dependency as a primary diagnosis.Excellent written and verbal communicationAssessment and case managementKnowledge of Microsoft Office toolsAbility to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibilityPREFERRED QUALIFICATIONS & SKILLSBasic knowledge of human servicesBilingual (Spanish/English) speaking preferred
