Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for the Brief Treatment - NJ Promise (Clinical Risk for Psychosis - CHRP) Department within Rutgers University Behavioral Health Care.The Habilitation Counselor provides community-based case management services for young people at clinically high risk for psychosis, including assessment, treatment/recovery planning, linkage, and coordination of care.Among the key duties of this position are the following:Provides case management and community support services to assist individuals (ages 10-25) and/or families to gain access to needed services.Provides crisis prevention, intervention, and resolution services.Provides training on the recognition of relapse triggers and strategies to avoid a relapse.Engages and establishes a working relationship with individuals with physical health, mental health, and substance use disorders.Provides specialized addiction treatment/co-occurring services.Collaborates effectively with internal and external stakeholders.Maintains appropriate records as mandated by policy and standards.Position Status

ABPQualificationsMinimum Education and Experience

Bachelor's Degree in a mental health field or an appropriate human services field plus one (1) year of related experience.Certifications/Licenses

All clinical licensing as required by discipline and New Jersey Division of Consumer Affairs.Active New Jersey certification/licensure for Social Work.Valid driver's license.Required Knowledge, Skills, and Abilities

Serves assigned shifts, which may include weekend, holiday and/or off-hour shifts.Provides on-call coverage as assigned.Ability to communicate effectively.Ability to focus on tasks despite interruptions.Preferred Qualifications

Certified Alcohol and Drug Counselor (CADC).Bilingual skills (English/Spanish).Experience working with high need individuals and community-based service provision.Equipment Utilized

Computer, Telephone, Xerox machine, Fax machine.Physical Demands and Work Environment

Must be able to sit for extended periods and document in an electronic medical record.Employee spends time driving, walking, standing, and talking with individuals in the community.Transport of individuals in state vehicles.Must be able to lift up to 20 pounds.Work environment is predominantly in community settings.Subject to unpredictable situations and crises.Occasionally exposed to individuals exhibiting assaultive behaviors.Exposure to blood borne pathogens requiring personal protective equipment.Special Conditions

Must possess own vehicle for outreach.
