Position Overview:

I. SCOPE OF ROLE:The Registered Nurse will meet with staff to coordinate shelter residents to receive ongoing treatment in the community, supervise LPN, Patient Navigator, and Health Educator, assist Psychiatric NursePractitioner as needed. He/she will Administer medications and document administration asrequired. Take vital signs, such as blood pressure, temperature and weight. Assist with diabetesmedication and treatment and take medical histories and enter information in computer systems. The RN will coordinate with Case Manager, Senior Case Manager and Director of Social Services to ensure proper care, nutrition and treatment plans are followed.II. ESSENTIAL DUTIES & RESPONSIBILITIES:Complete medical assessments and Communicable Disease Risk Assessments within mandatedtime framesCoordinate delivery and reception of medications with pharmacyElectronic Health Record data entry and maintenanceTrack all medications received and administered as well as coordinate return of unusedmedications to pharmacyObserve self-administration of medications for detoxification according to established protocolsRefer for required medical servicesCarry out urine screenings in accordance with program policyProvide education and consultation to individual clients and/or groups on their health needsAssess clients medical record and coordinate medical care with medical director and/or outsideclinicsAssure clients compliance with prescribe medical regimenComplete timely and accurate documentation on medical services renderedKeep supervisor informed of all relevant information regarding the residents and the programOther duties as requested by supervisorQualifications:III. REQUIREMENTSREQUIRED EDUCATION AND EXPERIENCENYS RN licenseKnowledge of chemical dependency as a primary diagnosis.
