Job DetailsJob Details:
We are seeking a dedicated and compassionate Patient Care Manager who is a Registered Nurse to join our dynamic healthcare team. This is a permanent, full-time position that involves providing top-tier medical care to our patients. Your role will encompass a wide range of tasks, including assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. You will be working in a diverse and fast-paced environment where no two days are the same. Responsibilities:
- Perform physical exams and health histories before making critical decisions.
- Provide health promotion, counseling, and education in a hospice or long term care facility
- Administer medications, wound care, and numerous other personalized interventions.
- Interpret patient information and make critical decisions about needed actions.
- Coordinate care, in collaboration with a wide array of healthcare professionals.
- Direct and supervise care delivered by other healthcare personnel like LPNs and nurse aides.
- Conduct research in support of improved practice and patient outcomes.
- Provide emotional support and compassionate care to patients and their families.
- Supervise home health aides when such personnel are part of the patient's care team.
- Administer palliative care as needed to keep the patient comfortable.
Qualifications:
- Must hold a current, valid RN license in the state of practice.
- A minimum of 3 years of nursing experience, preferably in a hospice or home health setting.
- Proven experience as a registered nurse.
- Excellent knowledge of nursing care methods and procedures in hospice or long term care facilities
- Excellent knowledge of emergency care.
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times.
- A team player with excellent communication and interpersonal skills.
- Responsible and compassionate.
- Outstanding organizational and multi-tasking skills.
- Patient with excellent problem-solving skills.
