Salary: $38 - $43 per hour

We're seeking a dynamic, compassionate, and patient-focused Permanent RN Liaison/Navigator to join our dedicated healthcare team. The successful candidate will play a pivotal role in providing a seamless transition of care for patients from hospital to home, nursing facility, or hospice. You'll work closely with patients, families, and healthcare providers, ensuring that the right care is provided at the right time in the right place. This role requires a deep understanding of clinical care, strong communication skills, and the ability to work autonomously while remaining part of a larger team.

Job Details

Qualifications:

  1. Current and unrestricted Registered Nurse (RN) license.
  2. 2+ years of experience in a clinical setting, preferably in a liaison or navigator role.
  3. Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  4. Basic Life Support (BLS) certification required.
  5. Exceptional communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare providers.
  6. Strong clinical knowledge, with a particular focus on hospice and palliative care.
  7. Ability to work independently and make critical decisions, while also working as part of a larger team.
  8. Proficiency in electronic medical records and other healthcare technology.
  9. Compassionate and patient-focused, with a strong commitment to providing the highest level of care.
