Position Summary:

This position will assist the Director of Reentry in developing and implementing a reentry project that will provide direct services to clients reentering the community from in-patient treatment and/or correctional facilities. The Lead Navigator will provide case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation, and participant hand off to a navigator for long-term peer support services. This position works in conjunction with the case management team, the assigned interdisciplinary team.

Essential Duties and Responsibilities:Mentor Navigators to optimize performance, continue growth and development, and assure professional standards are met and maintained.Develop individual treatment plans; coordinate with service providers, determine community resources needed for clients and families.Provide in-the-field resources and referrals and limited peer coaching support.Ensure participants are aware of program resources and services that are available to them and how participation in this program could potentially impact their involvement in the justice system.Ensure that applicable procedures are followed and that clear, detailed records are kept.Understand and ensure the Recovery Navigator Uniform Program Standards are met.Support the de-escalation and stabilization of participants when necessary and establish appropriate relationships with crisis services.Provide information on a timely basis and in a usable form to others who need to act on it.Perform ongoing monitoring and progress reviews to assess client’s progress and adjust case plans as needed.Participate in meetings, trainings, treatment planning, and case staffing as required.Develop rapport with clients to facilitate clients’ movement toward self-sufficiency by ensuring that clients remain consistent with their treatment.Work with and become knowledgeable about representatives across systems that impact participants, including the healthcare system, the behavioral health system, the criminal justice system, etc.Assist in researching and drafting policies, procedures, protocols and forms.Establish guidelines, best practices, client tracking, and expectations for the program and navigators.Understand and utilize best practices with progressive engagement, trauma-informed care, motivational interviewing, and other necessary skills suited to engaging potential program participants.Provide outreach at in-patient treatment and correctional facilities for clients who can be diverted into the reentry project.Provide individual case management for clients and participate in wraparound case management, which includes referral and follow-through with the Culture Program, Language Program, outpatient treatment, other social service programs, justice services, educational programs and workforce programs.Coordinate peer-to-peer mentorships, small talking circles and life skills classes for clients with other staff.Establish and maintain client case files, ensuring current quality standards, as required by Director.Work collaboratively and respectfully with staff, Social Services staff, Administration, support staff and various institutions and treatment agencies.Adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, and program policies (i.e., critical incident reporting, HIPAA, mandated reporting, etc.)Perform other duties as assigned.

Supervisory Responsibilities:

Directly supervises Reentry Navigator position and any others designated by the Director. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Qualifications and Requirements:

Education and/or Experience:

An associate’s degree with a background in social work, sociology, chemical dependency, human services or social sciences and three or more years of relevant experience and/or an equivalent combination of education and experience is required. Experience with Department of Justice policies and programs preferred. Must demonstrate the ability to provide program intake, advocate for and procure client services. Intermediate computer skills in Word and Excel, and Outlook required. Must be able to effectively and accurately utilize client data management systems.

Other:

Demonstrate leadership ability and has experience and willingness to lead change. Must have strong customer service skills with the ability to present information in one-on-one and small group situations to internal and/or external clients. Understand interdisciplinary team concepts. Demonstrates knowledge and understanding of HIPAA and other relevant Puyallup Tribal policies. Effectively incorporates confidentiality into daily work habits. Demonstrates knowledge and effectively applies pertinent Reentry regulatory standards to the work to assure compliance with such standards. Conforms to scheduled work hours; follows established department norms for attendance, punctuality and dependability so as not to negatively impact client care. Ability to work any day of the week, including evenings, weekends, and holidays.

Language Skills:

Effectively conveys and receives ideas, information and directions. Demonstrates good verbal and written communication. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Licenses or Certificates:

Must acceptably pass Washington State Department of Social and Health Services criminal history and background inquiry prior to and periodically throughout employment. Must have and maintain a Valid Washington State Driver’s License and proof of insurance. Driver’s License must not contain restrictions that would otherwise prohibit the employee from driving a GSA vehicle. Other Requirements: A minimum of two years documented recovery from substance or chemical dependency issues; or no prior history/diagnosis of chemical misuse, abuse or dependency is preferred. Knowledge of local, state, federal and private programs resources. Ability to work effectively with diverse individuals, groups, and communities. Sensitive to and knowledge of Indian customs, traditions and culture. Attends in-service training as appropriate. Maintain appropriate and ethical boundaries with clients. Uses effective teaching and modeling methods while interacting with clients. Successfully works with others to achieve desired results by working and communicating in an understanding, supportive and flexible way. Ability to work in crisis situations. Commitment to respecting the confidential nature of the program. Must be willing to attend trainings for professional development. A documentable background in motivational interviewing for substance abuse and a working knowledge of cognitive behavioral therapy.
